Case Management 

Are you aware of all of the resources you have available to help your family? Our clinic can provide you with a case manager who will be your advocate in getting your life in order.

Case management works to advocate for client needs by linking clients with appropriate providers and resources through assessment, planning, and care coordination, to help individuals and families achieve wellness and independence.

Case managers assess your needs and collaborate with you on what specific goals you want achieved. They then become a connection to resources such as housing, medical facilities, and employment opportunities. They do this by coordinating with state funded services to find what will best benefit your situation. They can’t do the work for you, but they will be there every step of the way until you can do it on your own.

Peer Support

Peer support links people living with a chronic condition. People with a common illness are able to share knowledge and experiences – including some that many health workers do not have.
Peer support is frequent, ongoing, accessible and flexible. Peer support can take many forms – phone calls, text messaging, group meetings, home visits, going for walks together, and even grocery shopping. It complements and enhances other health care services by creating the emotional, social and practical assistance necessary for managing the disease and staying healthy. Some core functions include: assistance in daily management, social and emotional support, linkages to clinical care and community resources, and ongoing support over an extended amount of time.

Certified Peer Support Specialists provide peer to peer support to adults experiencing mental health challenges. Peer support specialists model recovery to clients, providing hope and social support through empowerment and advocacy, to improve or maintain the client’s wellness and stability in their home and community.

Family Support

Families want the best for their children, to nurture their growth and development. Families with children who have special needs want the same for their children, but often face additional challenges, including great stress, social isolation, and financial strain. Families may become overwhelmed, and find it difficult navigating the service system for their child, and to find social and emotional support for themselves. "Family Support" helps families access a broad array of supports and services, including formal supports (such as paid respite care) and informal supports (such as parent-to-parent connections) and a community system of services that promote the well-being of families and their children with special needs.

​CBRS/ PSR

Community Based Rehabilitation (CBRS): Formally known as Psychosocial Rehabilitation (PSR), is intended to help participants manage their mental health effectively and appropriately while maintaining the highest level of independence possible. It is designed to improve the participant’s ability to manage issues in each focus area, through one-on-one instruction. CBRS emphasizes skills training and community reintegration.

Skills Building/Community Based Rehabilitation Services specialists are certified to teach individuals skills to improve or maintain their wellness and stability in their home, community, and other social environments. Certified specialists help clients identify their goals and provide education in a variety of skills areas including problem solving, social, communication, financial/budget planning, and independent living.

Respite Care

Everyone needs a break. Respite care provides short-term relief for primary caregivers, while the patient continues to receive care in a safe environment. The physical, emotional and financial demands for caring from someone with a disability can be overwhelming. Through respite care you can take care of yourself, while we care for your loved one. 
This service is offered at our Idaho Falls location, or at your own home.

Day Treatment

Day treatment is a level of care between outpatient, such as a therapist’s office, and inpatient, such as a hospital or residential facility. Youth and families engage in an intensive therapeutic setting and seek to make progress with emotional and behavioral stability, family conflict, self-harm behaviors, academic failure, and interpersonal problems due to emotional health challenges.
